Need a second revision

I need to have a second revision, and I have been thinking about it off and on, but as the size of my bridge narrows down to my dislike and I feel that my tip is getting smaller on one side I feel that I really need to make this decision, I had been considering Dr Toriumi I like most his pictures on his web site and the review site:

some of the negative stories really scare me, although I am sure there are a lot of exaggeration and anger mixed with them.

plus the only other Dr that I can partial trust after reading his journals is Dr Guyuron and as I compared their Journal publication, techniques and pictures, it seem like Toriumi might be technically a head except that aesthetically he might fall behind a bit, can anyone please give me some advice or share their thoughts on these two surgeons with me.

    Hi there! Sorry for the slight delay in getting this post published. I agree that the negative stories out there about Dr. Toriumi are rather concerning. They are enough to keep me away (but that’s just me!). That said, I also agree that some of it may be over-exaggeration. However, it is my opinion–based on what I have seen–that he does tend to make the nose larger than other surgeons would. I’m not particularly fond of his aesthetics. However, if you are looking for a larger nose (and it seems like you are), he may be right for you. If you feel uncomfortable–even the slightest bit, don’t do it. IMHO, Dr. Guyuron seems like a safer bet between the two. He does like to augment as well, but I haven’t seen him go overboard like T. He’s definitely worth a consult. Keep us posted on what you decide!! When are you thinking about having your surgery?

    no problem, I appriciate that you have formed this form as there seems to be no longer be any other reliable forms out there. I know that the unhappy T patients are concerning specialy since many of them have the same area of concer, but at the same time I have been in touch with few of his happy patients and I liked their results and that is why I am not so confused. I know some of the unhappy patients went on having revision and I know of two of them that I did like their T nose and would have not revised it considering the risk. The thing is that I do not like big noses I just do not want tiny tiny nose either. I think there is very little chance for thin skin patients to end up with big noses from him, I have seen 4 results on thin skin patients and they were not big\wider compared to thicker skin patients. I hope I am not wrong on my observation;( I have very hard time finding patients of Dr Guyuron, I am only afraid that he will not graft enough or use the right material and I will be back where I am, do you know much info on him or do you know of any of his patients?

    That is a very interesting observation you made with T patients–that he makes the noses larger in thick skin patients. I never noticed that. It makes sense, because docs really can’t make the nose too big for patients with thin skin (if they do, they risk damaging the skin, necrosis and other problems). Regarding Dr. Guyuron, I think he’s a big ear cartilage grafter (if you have no septal cartilage left, that is). I think the concern you have about him is a common concern that you should have about everyone–even Toriumi. With Toriumi, you should be concerned whether rib is a material you even need in your nose. I have been in direct contact with only one of his patients, though I have seen his work on forums over the years. I can try to dig up that one patient’s email address. I will try to find it and email it to you–perhaps she still checks that email address and will respond to you. Is the yahoo email you included on the comment form valid or was it a fake one? LOL. No need to type it again if it is valid–just let me know if it is–and I will send you an email late tonight.

    Hi Admin, Thank you for sending the address to me I got it, I actualy had talked to her before and she ws very nice in sharing her information. Althought it was very early on after her surgery so I would like to know how she likes her results after the swelling is gone now and hopefuly she is happy:)
    I do not want to mislead any one but based on pictures of T I have seen he does seem not to make the thin skin patients as big, he even told me he could not even if he wanted too(some time the bone structure and thin skin does not allow for that and it will be risky) if you remember there was a gril name Rachael who really wanted her tip build up longer and T did not do that and unlike most unhappy T patient her complain was that he wanted longer bigger tip;( also Natasha had thin skin and she never complined about the size but the angle of the tip, and I know of few other thin skin patients who are happy and the thin skin worked in their favor.
    you are right the one true question for me do I really need rib? that is so hard to figure out, but I have seen few people who needed work on the bridge like making it straighter, or stronger or wider, and after two ear revision they had to opt for rib, becasue ear did not hold good enough over time… Even Natasha told me she did not regret using rib in her bridge becasue now it is stronger and straighter(although she wanted her tip angle higher\cutre) and now she got that as well:) so that is the hardest question for me to answer I hate to go through the long surgery of rib if I could get away with ear. my fear with ear is that it will not hold up over time and I will back with same issues and more scar tissue. also I looked at Guryuron photos only few on his site;( they do not look that complicated at all but the results are natrual, I just wish there were more patients of him around that are few years post opt to know how they think of their results long term.
